Preparing for your stay

Please bring the following information with you to the Hospital on the day of your admission. This will help the admission process go smoothly.

  • Complete list of all the prescription and medications that you are currently taking
  • All medical Reports.
  • Medical insurance information, including insurance cards, Photo ID proof (Government-issued photo ID, such as a driver's license or passport) and other documentation required by your insurer
  • List of telephone numbers of immediate family members to call, if necessary
  • Personal toiletries, such as a comb, brush, your own shampoo, soap, toothbrush, and toothpaste
  • Your own Towels, Hand towels, napkins and slippers.

Our Policies, Systems and Processes

For admission contact the Front Desk Executive (FOE).

  • You will require an 'Admit advise Slip' from a doctor of Agadi Hospital to register for admission.
  • Get a permanent hospital registration number and card from the FOE.
  • Place your request with the admission assistant to select a room of your preference. All efforts will be made to provide you with the same room. In case of non-availability of requested category, an alternate room will be allotted and you will be shifted to the room of your choice as and when it becomes available.
  • The Ward Attender will escort you to the allotted room.

For room reservations, please call the receptionist 3 days prior to date of admission. We will try our best to provide you with the room of your preference.

It is provided to private ward patients. This kit contains:

  • Mineral water bottle
  • Thermometer
  • Shampoo
  • Soap
  • Napkin

Visitor Guidelines

  • In the better interest of the patient, to control the crowd, noise and infection level, only one attendant is allowed to stay with the patient and
  • During the visiting hours only 2 visitors are allowed at a time, please cooperate.
  • In the female ward only female attendants are allowed to stay.
  • Visitor Hours: 4.00 PM to 7:00PM on weekdays, 10.30 am to 12.30 pm & 4.00pm to 7.00pm on Sundays.
  • Please note that certain units may have a slightly modified visiting schedule.

Senior Citizen Concession

For senior citizens aged 65 years and above, we have a special concession protocol. To avail this benefit, please fill in the senior citizen application form with passport size photograph and date of birth proof. For any further queries on this, please contact the front office.

Other details

  • Patient will be allotted only one bed at any point of time. Only in special circumstances on request, they will be allotted additional bed, subject to availability.
  • During Hospitalisation, if the requested deposit amount is not paid, the hospital reserves the right to transfer or discharge the patient on administrative grounds.
  • For details of deposits, please refer to the schedule of charges.
  • During your stay if you have not received any intimation of the amount accrued to your account, please contact the OPD Manager/ PRO.
  • Hospital tariff is available in the front office and are subject to change without notice.
  • No beddings would be provided for the attenders nor would be allowed from outside.
  • We request one attendant to be accompanied by the patient during admission/ daycare.
  • The right to discharge the patient is with the doctors.
